Self-care tracker/reminder sheet A5

This “self-care sheet” reminder/communication card helps the owner to remember what activities or items provide comfort or allows them to rest. It can also communicate to others around of these activities and how they can help the owner of the card.

The card is laminated to make it water resistant and has the measurements of around an A5 piece of paper.

My cards come with the standard red heading but the colour can be changed on request.
